Delivering integrated care

An ageing society, a rise in chronic diseases, increased demand for primary care and personalised life-long care as well as resource limitations are challenging the sustainability of Europe’s health systems. Health demands and needs are evolving, as are the societies and innovations aimed at improving the performance of healthcare systems. Truly patient–centred systems need to be developed around the individual patient and replace the current healthcare focus on acute care episodes with a system that emphasizes prevention

To provide solutions to the growing demand for improved patient experience and health outcomes of multimorbid and long-term care patients,  Health First Europe developed the HFE Model on Community Care in 2013. It is a horizontal model for integrated care with the objective of fostering innovation and breaking down silos that prevent citizens from accessing an integrated care system. Since then, we have been working to foster policies that integrate patient care throughout patients’ lifetimes.
